Payment and Fee
Procedures: We accept most insurance types, including Employee Assistance
Programs (EAP) insurance. Some of the insurance panels we represent include:
Anthem, Blue Cross/Blue Shield (BC/BS), Cigna, Employee Benefits Administrators
(eba), Humana, Magellan, Tricare Prime and Standard, United Behavioral Health (UBH),
United Healthcare and many more. EAP insurances covered include: Bensinger-Dupont
Associates (bda), Ceridian (Military One Source), Magellan EAP, and Value
Options. If your insurance plan is not listed, please contact us! We do not
provide Medicaid services, at this time, unless it is tied into Medicare.
Some insurance plans will cover the
full amount for services rendered; however, most plans will include a deductible
and/or co-pay. Deductibles apply especially during the first half of each
year. For those potential clients who fail to have insurance coverage we
provide a sliding scale fee based upon income and number of dependents, but this
only applies to appointments after the initial assessment. Clients, with no
insurance, will be charged the full assessment charge. For those clients who
are required by the court system (or other programs) to attend counseling a
reporting fee may apply. This will entail the therapist being requested to send
documentation of attendance and/or performance to the referring party. Clients
needing a full written analysis of the assessment or a lengthy progress report,
along with recommendations for treatment, will include an additional charge
because of the extended amount of time involved by the clinician. Full
assessments may often include two or more sessions depending upon clinician
recommendation.
Office Protocol requires clients to complete a packet of documents prior to the initial treatment session. This process typically takes up to 15 minutes to complete. Individuals who tend to be challenged by their reading or language may wish to arrive 30 minutes early. Any and all questions will be addressed as thoroughly as possible.
Treatment sessions typically start at or around the top of the hour and last 45 minutes to 55 minutes. Office hours are flexed to accommodate the needs of our clientele. Wednesdays and Thursdays offer late appointments compared to other days; Saturday appointments are offered by Bruce Hey, but these time slots are often filled early. The office is closed on Sundays and Mondays, though each clinician may choose to adjust their own work schedule beyond the posted hours depending on client’s needs and their own personal/family schedules. Clients are requested to give 24 hour notice if they will be unable to attend.
Information of client’s treatment is confidential. Shared information requires written authorization by the client or, if under 18 years of age, the client’s parent or guardian. At times, information may be shared without client’s permission: according to the law, when state or federal mandates exist – (here are a few examples) if someone is in immediate danger, if a judge gives written approval for records to be disclosed through subpoena, or if the provider is demonstrating due diligence in collecting fees not otherwise having been paid by either the insurance company or by the client.
